- You can fill it out before or after speaking with someone at the clinic
- It helps us understand your symptoms and medical history
- Particularly useful if you’re unsure which professional you need
- Please indicate if you already know which Clinical Team member you’d like to see
After this form is received, a member of our Patient Liaison Team will contact you by email or telephone within 2 business days to schedule your appointment.
Please note: we are not currently treating new patients under the age of 18.
Opening hours are 9:00am to 5:00pm, Monday to Friday (excluding bank holidays).
