While under care at Breakspear Hospital, you may be provided with a list of all supplements prescribed for you during your stay and maintenance treatment after discharge.
Please allow 7 days from the receipt of Pharmacy order.
How to order from the Pharmacy
If you are paying for your own prescription, there are two ways to submit your Pharmacy orders. (If you are covered by the NHS, skip down to the section "How to reorder from the Pharmacy through the NHS".)
1) Visit www.breakspearmedical.com/shop
- Register online (your membership application will be processed within 3 working days).
- Once you are fully registered, you can search, view, order and pay by credit/debit card securely online anytime of the day or night.
- We are offering free postage and handling for all orders placed and paid for online until 30 June 2010.
- Prescription only medicines (POMs) are not available online. Please contact Pharmacy directly 01442 261 333 ext 291 to order.
2) Complete a (printed out) Pharmacy Order Form
- Print out and complete the pharmacy order form. Make sure to state the full name and address of where the order is to be posted.
- Fax the order to +44 (0) 1442 266 388 or mail to the Hospital, attention "Pharmacy" or drop the form off at the Hospital.
- The Pharmacy will process the order and send it to you as soon as possible together with the invoice.
How to reorder from the Pharmacy through the NHS:
- Complete the pharmacy order form. Make sure to state the full name and address of where the order is to be posted.
- Take the pharmacy order form to your NHS GP for him/her to complete a prescription.
- Take your pharmacy order form and prescription to your pharmacy/chemist.
- The Pharmacist must sign and stamp the pharmacy order form and send this to our Pharmacy.
- Our Pharmacy will process the order and send the order to your chemist/pharmacy or directly to you, if agreed by the chemist.
Note: Some items may not be available on the NHS prescriptions and must be bought privately.
