Re-ordering

If you are a registered patient or practitioner at Breakspear Medical Group, you can register and visit our online shop www.breakspearstore.co.uk to view and order products online.

Alternatively, if you are a patient of Breakspear or a consulting physician and wish to re-order pharmacy products, you can use either our Pharmacy PDF Order Form, which can be emailed directly or print out and complete a Pharmacy Order Form and fax to +44 (0) 1442 266 388 or post to the clinic, attention "Pharmacy".
