Chronic fatigue (CFS), also referred to as epidemic myalgia, myalgic encephalomyelitis (ME), chronic viral syndrome and post viral fatigue syndrome, is a complex clinical syndrome of uncertain ætology. It is characterized by incapacitating fatigue, neurological problems, and a variety of symptoms that can resemble many other illnesses. Physiologically, it is probably a disease of immune activation, mediated through the elaboration of alpha interferon. See Chronic Fatigue & Myalgic Encephalomyelitis for more information.
The most important part of the diagnostic work up of this syndrome is the exclusion of other causes of fatigue such as cancer, autoimmune disease and chronic bacterial infection. The second is the recognition of the symptom complex.
Clinical examination often reveals enlarged, tender lymph nodes in the left supraclavicular fossa. In addition, patients have increased reflexes and often an inability to maintain the Romberg stance position.
